In Western Australia, children can start their education in Kindergarten (kindergarten students at USPS will need to re-apply for Pre-Primary they are not automatically rolled over). Compulsory schooling starts the following year in Pre-Primary.
You need to apply to enrol your children in school for 2025 if they are:
- starting Kindergarten – 4 years old by 30 June 2021
- starting Pre-primary, the first year of compulsory school – 5 years old by 30 June 2020

Upper Swan Primary School is located on Great Northern Highway in Upper Swan and is a local area intake school. If unsure if you reside in our local intake area please click on address check for local intake. Families that do not reside within the catchment area are welcome to complete an application form. Approval will be at the discretion of the Principal based on class numbers. Application to enrol at Upper Swan Primary School
Please complete and return the following forms to the school office or by email UpperSwan.PS.Enrolments@education.wa.edu.au to enrol your child/children at Upper Swan Primary School.
All applications must have the below documents 1-4. Any incomplete applications will not be accepted.

1. Application form

2. Copy of your child's Birth Certificate
If your child is born in Australia but both parents are born overseas, we will require a copy of the child’s Australian passport or parents’ citizenship documents to determine if the child is eligible for Australian citizen. For more information on this please refer to the Department of Home Affairs website.
3. Proof of your Residential Address
You will need to provide evidence to dertermine the student's usual place of residence.
You must provide;
1 copy of proof of home ownership, such as rates notice or lease agreement; and
1 copy of additional information, such as driver's licence, a utilities account, home insurance or statutory declaration.
Please note, your application may be declined if evidence of the student's usual place of residence is insufficient, false or misleading.

Please advise the Administration Officer when submitting your application form. This will ensure we have all the correct procedures in place to assist your child.
6. Access Restrictions
Parents/carers are reminded that the School Education Act 1999 obliges parents/carers to inform schools of any family court orders or other orders which are applicable to their children. Any changes to orders must be provided to the school as soon as possible.

Once your application has been accepted you will then be required to provide your child's Immunisation History Statement to proceed further with enrolment form.
On 22 July 2019, changes to the Public Health Act 2016 (WA) and the School Education Act 1999 (WA), came into effect that will better protect young and vulnerable WA children and the wider community from vaccine-preventable diseases.
Under these new changes, a child’s immunisation status must be ‘up-to-date’ according to their AIR Immunisation History Statement and must be dated within 2 months of the date of enrolment form completion.
