Welcome to Upper Swan Primary School

 

Upper Swan Primary School caters for students from Kindergarten to Year 6.
Please see below for key information about enrolments and starting school.


Starting School

In Western Australia, children can start in Kindergarten (non-compulsory) before moving into Pre-Primary, the first year of compulsory schooling.

At Upper Swan PS, Kindergarten students must re-apply for Pre-Primary — enrolments do not automatically roll over.

You’ll need to apply to enrol for 2026 if your child is:

  • Starting Kindergarten – turns 4 by 30 June 2022
  • Starting Pre-Primary – turns 5 by 30 June 2021

Local Intake Area
Our school is located on Great Northern Highway, Upper Swan, and has a local intake area. You can check whether your address falls within the intake area here: Local Intake Area Map.

Families living outside the catchment are still welcome to apply. Acceptance is at the Principal’s discretion, based on available class numbers. For more information, see Enrolment in Public Schools Procedures.

How to Apply

Application Checklist

 

1. Application Form

  • Download here or collect from the school Front Office.

2. Birth Certificate

  • Provide a copy of your child’s Birth Certificate.

  • If your child was born in Australia but both parents were born overseas, also provide a copy of the child’s Australian passport or the parents’ citizenship documents.

  • This confirms eligibility for Australian citizenship.

3. Proof of Residential Address

You must provide:

  • One document showing proof of home ownership or tenancy (e.g. rates notice, lease agreement), and

  • One supporting document (e.g. driver’s licence, utility bill, home insurance, or statutory declaration).

⚠️ Applications may be declined if the evidence of your child’s usual place of residence is incomplete, false, or misleading.

 

4. Latest School Report

  • For Pre-Primary to Year 6 students only.

5. Health or Special Needs

  • Inform the Front Office when submitting the application form so the school can arrange appropriate support.

6. Access Restrictions

  • Provide any family court orders or other legal documents affecting your child.

  • Updates to orders must be provided immediately.

Immunisation History Statement (AIR) -  To be provided with enrolment form.

Kindergarten:

  • Once your application is accepted, you must provide your child’s AIR Immunisation History Statement.

  • The statement must be ‘up-to-date’ and dated within 2 months of enrolment form completion.

  • This is required under the Public Health Act 2016 (WA) and School Education Act 1999 (WA) to protect children and the wider community from vaccine-preventable diseases.

Pre-Primary to Year 6:

  • An up-to-date AIR Immunisation History Statement, dated within 2 months of enrolment form completion, is also required.
